Ledipasvir是丙型肝炎病毒复制的抑制剂,其IC50值为141 nM[1]。
丙型肝炎病毒(HCV)是一种有包膜的(+)单链RNA病毒,可以引起丙型肝炎以及某些癌症淋巴瘤。病毒RNA通过HIV RNA依赖性RNA聚合酶在宿主细胞中复制,然后组装成病毒粒子。锌-结合磷蛋白NS5A由HCV基因组翻译而得,在病毒复制过程中起着重要作用。它可以充当NS5A RNA复制系统的转录激活因子以及病毒组装的介质。
Ledipasvir是HCV NS5A蛋白特异性的抑制剂,抑制HCV亚基因组复制子系统中的HCV复制。NS5A复制复合体抑制剂是新型抗病毒因子,可用于治疗HCV。通常情况下,相对于靶向NS3解旋酶和NS5B RNA聚合酶的传统HCV复制抑制剂,上述抑制剂具有较高的效力以及较低的病毒耐药性。NS5A抑制剂结合靠近N末端结构域1的NS5A二聚体界面,直接或变构扭曲二聚体的结合,从而扰乱NS5A在HCV RNA复制中的功能[2]。当使用JFH1/3a-NS5A杂交复制子以评估对NS5A的易感性时,另一种抑制剂DCV比Ledipasvir显示出更高的效力。此外,NS5A-A30K和-Y93H突变体对Ledipasvir显示出较低的敏感性(EC50值分别为1770 nM和4300 nM)[1]。
临床试验表明,Ledipasvir耐受性良好,HCV RNA最大减少值的中值范围为2.3 log10 IU/ml ~ 3.3 log10 IU/ml。Emax模型同样表明,对于基因型1a,30 mg的Ledipasvir于给药后第3天使HCV RNA减少值达最大(>95%)。此外,相对于基因型1a,HCV RNA在基因型1b中更持久[3]。
